Celiac Disease Is the Autoimmune Disease That Comes With a Map

In 2014, a biopsy handed me a celiac diagnosis. I thought the fix was gluten. I was wrong about the category.

Celiac is an autoimmune disease. It shares a family with type 1 diabetes, rheumatoid arthritis, and lupus. Gluten sets off an immune response, and the consequences reach far beyond a stomachache. I read studies because I have to. I am not a doctor, so this is the view from our kitchen.

How a Dutch Pediatrician Found the Trigger

Aretaeus of Cappadocia wrote about the condition in the first or second century AD, using the word koiliakos, from the Greek koelia, meaning abdomen. He recorded patients whose food passed through them without being absorbed. Samuel Gee followed with a careful clinical description in 1888 and admitted he could not identify the cause.

For centuries, doctors cycled through bland diets and rest. By the early 1900s, bananas became the standard recommendation, following the work of pediatrician Sidney Haas. The banana diet helped some children gain weight. It never touched the mechanism.

Willem-Karel Dicke supplied the evidence during the Dutch famine of 1944 and 1945. He worked as a pediatrician in the Netherlands and noticed that children on his celiac ward fared better when wheat was scarce. He tracked their weights through the war. When bread returned after Swedish relief planes dropped supplies in the spring of 1945, the same children worsened within days. Dicke published those observations in his 1950 doctoral thesis.

Researchers identified the gliadin fraction of gluten as the problem in the 1950s. A disease that had been recognized since antiquity and fed bananas for decades turned out to be an autoimmune attack with a defined trigger.

What Makes Celiac Different From Other Autoimmune Diseases

Most autoimmune conditions keep their triggers hidden. Celiac has a defined one.

The Known Variables

  • The trigger: gluten, specifically gliadin fragments.
  • The target: tissue transglutaminase, an enzyme in the intestinal lining that the immune system mistakes for a threat.
  • The genes: HLA-DQ2 and HLA-DQ8.

A 2018 Lancet review put global prevalence of celiac disease at about 1 percent. Genetic studies consistently find around 95 percent of people with celiac carry at least one of these gene variants, yet 30 to 40 percent of the general population carries them too, and most of those people never develop the disease.

Type 1 diabetes has a partly known target: insulin and other proteins the pancreas makes. Multiple sclerosis researchers still debate which protein the immune system attacks first. People with lupus and rheumatoid arthritis produce many autoantibodies. Celiac sits apart because researchers can identify the trigger directly in food.

That clarity is why immunologists use celiac as a model disease. When all three variables are known, a condition becomes a reference for studying how autoimmunity starts, persists, and quiets. Some research programs have studied celiac for decades to understand how the immune system tells self from non-self, how intestinal permeability shifts the immune response, and whether removing a known trigger dampens the attack over time.

The Gluten-Free Aisle Made Everything Murkier

Researchers mapped celiac's trigger, genes, and target around the same time gluten-free eating became a mainstream trend. In the 2000s, wellness books, celebrity endorsements, and food companies searching for the next restricted product pushed gluten-free options into every grocery store. When that trend hit, my family gained useful products and confusing conversations.

A decade ago, finding gluten-free noodles, bread, and seasoning with a readable label felt like a scavenger hunt. Now the phrase gluten-free blurs between medical necessity and optional preference. That gap explains why cross-contact education matters. People who avoid gluten by preference can sometimes tolerate trace amounts. Kolbie was diagnosed with type 1 diabetes in 2020, then celiac disease shortly after. The two conditions co-occur more often than chance would predict. Studies put the prevalence of celiac among people with type 1 diabetes at five to ten percent, compared with about one percent in the general population.

Understanding the map helps me advocate. When Kolbie's school asks about a shared snack, I explain that celiac is an immune reaction with measurable antibodies, which separates it from a digestive sensitivity or a preference. I point to her written care plan and the gluten-free diet that sits alongside her insulin pump as part of managing the disease.

Because celiac's trigger, genes, and target are known, celiac anchors a large body of research into how autoimmunity develops and how it can be moderated. Scientists have used that work to study molecular mimicry and intestinal permeability in other autoimmune conditions. That research is early. It does not change what I put on our table. It changes how I explain our family to the world.
