Why Your Prescription Label Still Won't Tell You if the Starch Is Wheat

When I fill a prescription for one of my kids, I read the tiny paper insert and usually find the word "starch" or "pregelatinized starch." The label doesn't say corn, potato, tapioca, or wheat. That gap has sent me through more phone trees than I care to count. This is the history of how wheat starch became a common pill ingredient, and why the rules that made food labels clearer never made the same trip to the pharmacy shelf.

Wheat Flour Was the Apothecary's Binder

Before factories made pills, pharmacists made them by hand. A pharmacist mixed powdered medicine with a sticky substance, rolled the mass into a long cylinder, and cut it into equal pieces. Wheat flour or wheat starch often served as that sticky substance because it was cheap, stable, and already sitting on the shelf. Some old pharmacy formularies list wheat flour among standard pill binders, right beside gum arabic and simple syrup. At the time, no one connected wheat to celiac symptoms. The condition existed, but the science hadn't caught up.

A Famine, a Pediatrician, and the First Clear Link

The link between wheat and celiac symptoms became clearer during World War II. Dutch pediatrician Willem-Karel Dicke had already noticed that some children with digestive problems improved on a banana diet. During the Dutch famine of 1944 and 1945, wheat and rye flour became scarce in the Netherlands. Dicke watched children with celiac symptoms improve while bread disappeared, then worsen when wheat flour returned. After the war, he and his colleagues identified the protein fraction in wheat, gliadin, as the trigger. This history shows how young the science is. The doctors who first tied wheat to celiac symptoms lived in an era when drug starch sources were the last thing anyone thought to record, and the patients they were trying to help had no way to read a label anyway.

Industrial Tablets Made Starch a Workhorse

By the 1950s, most medicines came from factories rather than pharmacy mortars. Tablet presses required ingredients that flowed evenly into the machine, compressed into a solid under pressure, and dissolved in the stomach at a predictable rate. Starch did all three. It worked as a filler to bulk up a small active dose, a binder to hold the tablet together, and a disintegrant to help it break apart after swallowing. Pregelatinized starch, which is starch cooked and dried before use, became a common excipient because it had the right binding and flow properties. The plant source didn't change how the drug worked, so manufacturers had little reason to switch or to spell it out on the label. A pill could contain wheat starch, and the patient would never know.

Food Labels Got Clearer While Drug Labels Stayed Put

Food labels became clearer because advocates pushed for decades and Congress eventually passed stronger rules. The Food Allergen Labeling and Consumer Protection Act, passed in 2004, required food labels to state when a product contains wheat among the top eight allergens. The FDA's gluten-free food labeling rule, effective in August 2014, set a limit of less than 20 parts per million for a gluten-free claim. Drug labels didn't follow. The FDA regulates drugs under a separate framework, and inactive ingredients like starch can be listed by name without identifying the plant source. In 2017, the FDA issued draft guidance recommending that drug manufacturers voluntarily disclose when a product contains gluten from wheat. A draft guidance is not a rule. Companies may follow it, ignore it, or follow it inconsistently. This is the divide families like mine keep hitting: food labels became more transparent while drug labels stayed stuck in the excipient era, and no one has to tell you whether the starch in your prescription came from corn or wheat.

What Those Label Words Actually Mean

The words on a drug label aren't random. "Starch" usually means a plant starch, but the source isn't given. "Pregelatinized starch" means the starch was heated to improve its binding behavior, and it still doesn't tell you the plant. "Sodium starch glycolate" is a modified starch used to help tablets disintegrate, and it can derive from corn, potato, or wheat. "Dextrates" and "dextrin" are sugar-based fillers that can come from corn or wheat. "Maltodextrin" in the U.S. is mostly corn, but wheat-derived maltodextrin exists. You can't know from the label alone. The only way to find out is to ask.

The Questions I Ask for Every New Prescription

Before our family starts a new medication, I call the manufacturer or ask the pharmacist to call. My list includes four questions:

  • Is the starch in this product derived from wheat, barley, or rye?
  • Has the finished product been tested for gluten?
  • If the starch is wheat, has it been processed to remove gluten?
  • Do you have a written statement about gluten content for this specific lot number?

I write down the date, the lot number, and the name of the person I spoke with. Pharmacists often get faster answers because they have direct contact with manufacturers. If a medication is needed right away, I ask the pharmacist to help me sort through the options with my prescriber. I don't change or stop a prescribed medicine on my own. That decision belongs to the doctor or pharmacist who knows the full picture.

The Quiet Work Families Do Behind the Label

The cost of this missing information is time. Each new prescription can mean a fifteen-minute phone call, a hold line, and a transfer from customer service to quality control. During cold and flu season, when chewable tablets and cough syrups cycle through the house, the calls multiply. Parents managing celiac disease already read every food label, scrub cutting boards, and coordinate with teachers. The medicine cabinet shouldn't be another research project.

When I look at that history, I see how we got here: wheat starch became a workhorse excipient before anyone knew to ask about gluten, and drug labeling rules never caught up with food labeling rules. Knowing that history doesn't make the phone calls shorter, but it makes the process less confusing. It also gives me patience for the work, because I understand why the answer isn't printed on the bottle.
