Why the Word Autoimmune Showed Up Late in Celiac Research

Kolbie's Type 1 Diabetes diagnosis came first. Celiac followed. I started reading backward after that, trying to understand why one child could collect two immune conditions and whether that pairing had a history. The history was there, and it changed how I think about the diagnosis.

Celiac has been recognized by name for more than a century. The autoimmune part took much longer.

A pediatrician described the illness decades before anyone knew the mechanism

Samuel Gee, a London pediatrician, gave a lecture in 1888 called "On the Coeliac Affection." He described children with chronic diarrhea, poor growth, and swollen bellies. Gee wrote that if a child recovered at all, recovery would come through diet. He did not know which part.

In the 1920s, Sidney Haas promoted a banana-based diet for children with celiac symptoms. Some kids improved. The bananas were not the reason they improved. The diet happened to exclude most gluten.

A wheat shortage gave doctors the first real clue

Willem-Karel Dicke, a pediatrician in The Hague, watched children with celiac symptoms improve when wheat flour became scarce during the winter of 1944 and 1945. When Swedish bread shipments arrived and wheat returned to their plates, symptoms returned too. Dicke and his colleagues published those observations shortly after the war. By the early 1950s, researchers had narrowed the trigger to gluten, a protein in wheat, barley, and rye.

That was the dietary mechanism. The immune mechanism took another four decades to define.

The word autoimmune entered the chart later than people assume

Scientists identified antibodies against gliadin, a component of gluten, in the 1960s. A 1997 paper in Nature Medicine identified tissue transglutaminase as the target of the autoimmune attack in celiac disease. Tissue transglutaminase is an enzyme in the gut lining. In celiac disease, the immune system treats that enzyme like an invader after gluten enters the picture. The attack damages the villi, the fingerlike structures that absorb nutrients. That antibody response is why celiac belongs in the autoimmune category. Celiac is not simply a food intolerance.

The genetics support this. Almost everyone with celiac carries one of two gene variants, HLA-DQ2 or HLA-DQ8. Many people without celiac carry those same variants. The genes are necessary but not enough. Something else has to tip the immune system into attack mode.

Type 1 Diabetes forced me to pay attention to the overlap

In our house, Kolbie's two diagnoses are not separate problems. They share genetic risk factors and immune dysregulation. The American Diabetes Association's Standards of Care report celiac disease in 1.6 to 16.4 percent of people with Type 1 Diabetes, depending on the study. The general population sits near 1 percent. That gap is why pediatric endocrinologists screen kids with Type 1 Diabetes for celiac, often with blood tests even when stomach symptoms are absent.

Before the autoimmune label made sense to me, I thought of celiac as a gut problem. The overlap with Type 1 Diabetes helped me see it as a systemic immune condition that can show up in different organs.

What the autoimmune label means for future options

The only management for celiac disease today is strict gluten avoidance. That fact has not changed. Because celiac is now understood as autoimmune, researchers are testing approaches that go beyond the diet.

Publicly registered trials include:

  • Enzymes that break down gluten before the immune system sees it
  • Vaccines meant to teach immune tolerance
  • Treatments targeting the tissue transglutaminase interaction directly

None have replaced the gluten-free diet. Some trials have failed in late stages. The diet remains the standard.

I read trial registries the way I used to read adoption paperwork, looking for something that would change a daily reality for Kolbie. The difference now is that I understand why a drug would need to act on the immune response, not just on digestion.

How this history shapes our kitchen

Understanding celiac as autoimmune changed how I handle a shared kitchen. Cross-contact is not a preference. A crumb on a cutting board can trigger an immune response even without immediate stomach symptoms. We keep dedicated toaster bags and labeled jars. Double-dipping is not allowed.

Label reading extends to seasonings and bouillon cubes, because wheat and soy can hide there. When Kolbie missed ramen, I could not find one with clean ingredients, so we made our own.
